ATODLEN 8Cynnwys hysbysiad am ddyfarnu i ddarparwr o dan y Broses Darparwr Mwyaf Addas
1. Datganiad bod dyfarniad wedi ei wneud yn dilyn y Broses Darparwr Mwyaf Addas.
2. Enw a chyfeirnod y contract.
3. Enw a chyfeiriad (naill ai swyddfa gofrestredig neu brif fan busnes) y darparwr y mae’r contract wedi ei ddyfarnu iddo.
4. Disgrifiad o’r gwasanaethau iechyd perthnasol y mae’r contract yn ymwneud â hwy, ac un neu ragor o’r codau GGG sydd fwyaf perthnasol i’r contract.
5. Gwerth oes amcangyfrifedig y contract.
6. Y dyddiadau y mae’r contract yn darparu ar gyfer darparu’r gwasanaethau iechyd perthnasol rhyngddynt a hyd y contract gan gynnwys estyniadau posibl y tu hwnt i’r cyfnod cychwynnol.

11. Gwybodaeth am sut y cafodd unrhyw wrthdaro buddiannau neu unrhyw wrthdaro buddiannau posibl ei reoli, neu sut y mae i’w reoli.
12. Unrhyw wybodaeth nad yw wedi cael ei chyhoeddi neu sydd wedi ei dal yn ôl fel arall o dan reoliad 23(1).
